Making a My Number （individual number） card

My Number card is a plastic card that registered residents can apply to receive.The front of the card contains your photo, name, address, date of birth and gender, so it can be used as a personal ID.

Your My Number is listed on the back, so it can be used to confirm your My Number digit numbers.

My Number card applications 

You can apply at the Civic Affairs Section on the first floor of the Urayasu City Hall or at the City Service Centers of Urayasu, Shin-Urayasu or Maihama Stations.

You will need your residence card or special permanent residence certificate.No fee required.

Your My Number card expires when your visa expires.

If you have applied to extend your visa, you can also extend your My Number card’s expiration date to the new expiration date.Please complete these procedures at city hall before your My Number card expires.

If your My Number card expires, you will need to apply to have it reissued.This requires a fee.
